2008 COLLEGIATE BASEBALL OF THE WEST PLAYER APPLICATION
Please read thoroughly before completing and mailing this form to our office: CBW is accepting player applications beginning December 1, 2007 for the Pacific Javelinas, Portland Titans and Seaside Wildcats teams for the 2008 collegiate summer season. Player fees are NON-REFUNDABLE once accepted to a roster. Players must post additional $100 uniform deposit by posting a credit card or $100 cash. Credit Card posted will not be charged or $100 cash refundable if complete uniform (jersey, pants, belt) is returned at end of participation for the summer.

-AFTER 2/1/08 - $800 by check, $825 VISA/MC
-ALL FEES MUST BE PAID IN FULL BY 5/31/08

*Teams will play 34-36 games between Northwest Collegiate League and tournament play. Player fee includes team travel and accommodations for two nights. Players are responsible for their own meal and entertainment expenses while on team travel.
This is an application, not a guarantee of being placed on a team. Please fill out application in full, including full payment. Payment can be made by check payable to CBW or by credit card, please list credit card payment info below. If not selected by CBW to participate the player fee will be refunded in full.

AGREEMENT: RELEASE & HOLD HARMLESS
THE UNDERSIGNED (“Participant”) is over the age of 18 and wishes to participate in the CBW Collegiate Summer Baseball organization (“CBW”), a non-professional collegiate summer baseball organization.
Participant understands that participating in “hardball’ baseball may result in injury or liability and Participant recognizes and assumes that risk.
Participant has been advised that CBW does not carry insurance for any injuries or liability that the participant may receive or incur while participating in CBW events.
Participant has been advised that if he/she desires insurance for liability and injuries received while participating in CBW events, then he/she must make his/her own arrangements for insurance.
For and in consideration of participating in CBW, Participant agrees as follows:
Participant releases and holds CBW harmless from any and all claims for injuries or liability he/she may receive or incur while participating in CBW whether or not based on the negligence of CBW.
Participant accepts sole and full responsibility for the cost of treatment of injury and defense of claims.

Player Code of Conduct
As a member of Collegiate Baseball of the West organization, I recognize that:
*Summer Collegiate baseball represents those things which are great about the sport of baseball and amateur athletics.
*I am an ambassador for my community, for baseball, for amateur athletics, for my team and for myself.
*The behavior of any single player (or group of players) can have an impact upon the team and its reputation in the community. It’s young people, and within the baseball community.
*I have an obligation to conduct myself on and off the field in ways which bring honor and distinction to myself, my teammates, this team, all of baseball and this community.
I therefore pledge that:
*My actions on and off the field will be positive and I will strive to be a model citizen, to help create a positive perception of myself, my team and baseball.
*I will show consideration for the general public; for other guests at my living accommodations and any hotel at which the team may stay while traveling; for others any time we are public.
*I pledge to do everything I can, on and off the field, to help create a positive perception of myself, my team and summer collegiate baseball.
I understand that:
*The management of CBW will have no tolerance for behavior which casts the team and baseball in an unfavorable light. The consequences will be:
My tenure as a member of CBW will come to an immediate end.
Other teams will be notified that I have been dropped from the roster and why.
My collegiate coach will be notified, outlining the reason for my departure.
A similar letter will be sent to my parents.
This action will be included in any reports which CBW may provide scouts from professional baseball. (I am aware that professional baseball organizations count off-field conduct as an important part of their scouting process on every prospect.)
